This form is for anyone wanting to apply to adopt one of the animals in our care. Please read the entire application and fill it out to the best of your knowledge. Once the form is submitted, if you have been selected for the next step in the adoption process someone will reach out to you to set up a meet and greet.

2- C.A.P.P. (Cash Assistance for People with Pets) forms can be found here.

This form is for low-income individuals who are looking for assistance with vet bills or animal care items. Please submit as much information as possible with your form so that we can best asses the situation and determine if we are able to be of assistance.

3- Volunteer forms can be found here.

If you are interested in volunteering please fill out a volunteer form and submit it through the link above. All volunteers must be 18 or older or have a legal guardian present at all times when on the property. Volunteers will be required to sign a liability waiver and attend a 1 hour volunteer orientation before allowed to complete volunteer assignments.

Please use the form above if you are requesting to admit any pet(s) into the rehoming program through Perfect Paws Animal Rescue. Our shelter only intakes cats and dogs. If you are looking to rehome any other breed of animal please refer to the Illinois Department of Agricultures website for a list of other rescues in the state.
